Housing Coordinator Position
Job Description

Reports to: Duties are performed under the direction and supervision of the Helpline Executive Director.

Salary: $2,000-$2,400/month

Benefit Package: 15% of base salary for retirement and/or health insurance and applied only to agency plan.

Job Summary: Position will provide case management for individuals and families who are homeless, living in shelter, transitioning to permanent housing or in danger of losing permanent housing. The purpose of this position is homelessness prevention. This will be accomplished by assisting families in accessing community and personal resources that will enable them to have successful living experiences in permanent housing. The Housing Coordinator will actively collaborate with the goals and objectives set forth by the Homeless Coalition and local agencies that serve the homeless and provide housing.

Supervisory Responsibilities: None

Essential Functions:

Provide case management services to homeless individuals and families, including those living in shelter;
Provide case management to those individuals and families transitioning into permanent housing and to those who are in danger of losing permanent housing;
Assess the needs of clients;
Develop and implement appropriate comprehensive individual service plans for clients;
Ensure all documentation is completed within required timelines;
Collaborate with community resources to assist clients;
Recruit and support community leaders and volunteers to provide services to clients (e.g. budgeting workshops, banking, homemaking skills, etc.);

Examples of Duties:

Maintain accurate and required records of individual client services;
Actively participate in Helpline staff meetings, Homeless Coalition meetings and other meetings as assigned by Executive Director;
Set up home visits with clients;
Assist clients in identifying community and personal resources;
Support clients in making necessary appointments with community resources and arrange transportation required for these appointments;
Arrange for workshops that might help clients with specific life skills;
Other duties of similar nature, responsibility and level of difficulty may be assigned.

Working Environment/Physical Abilities:

Work is performed both in the office and in the field;
Utilize general and specialized office equipment, including computers;
Requires driving in varied weather conditions;
Able to walk, negotiate stairs, stand and sit for extended periods of time.

Knowledge and Abilities:

Possess knowledge and ability to do client intake requiring both assessment of client history concerning both short- and long-term needs;
Possess knowledge and ability to implement case management principles;
Possess knowledge of community resources;
Possess effective oral and written communication skills;
Maintain working knowledge of and adherence to all agency policies and procedures;
Maintain client confidentiality;
Participate as a team member in a rapidly changing behavioral health care setting.

Qualifications:

Bachelor’s degree in Social Work, Social Services or related field is required.
Three (3) years non-profit direct services experience and/or a Master of Social Work preferred.

Licenses and Other Requirements:

Obtain and maintain current First Aide and CPR certificate.
Maintain valid driver’s license, acceptable driving record and vehicle insurance coverage.
Access to a suitable vehicle during working hours.
